General Data Protection Regulation(GDPR) Policy

General Data Protection Regulation (GDPR) is concerned about the information I collect, store and share. Below is my GDPR policy. At our first meeting I will request some personal details

Personal Information

• Name
• Age
• Date of birth
• Gender
• Relationships
• Occupation
• Medical conditions relevant to counselling
• Prescribed medication
• Past counselling history
• Issues you are experiencing currently
• Session summary

Contact details

• Address
• Telephone/mobile number
• Email address
• I will seek your permission to contact you by text, email or leave voice messages as needed.

How I will store your personal information

• Paper: hand written notes. See below
• Smartphone: I will hold your contact details on my phone using your first name and surname initial only.
• Email address: will be stored in my email account (info@worcestershirecounselling.co.uk)
• Website: none of your personal information is held on my website, other than briefly to collect and send to my email account above.

Processing /Sharing information

• In accordance with the British Association of Counsellors and Psychotherapists (Bacp) I will seek monthly Supervision to process my working practice. My Supervisor will not know you personally or professionally. I will refer to you by your first name only and may share information to develop my professional practice only.
• In the event of my death or serious illness my Therapeutic Executor will access your name and contact details so that they may contact you if we are still working together.

In the case of an Emergency

• If your physical/ mental health is at risk I may share your contact details with an emergency healthcare service. I will always make every effort to discuss with you and seek your permission to do so.

Erasing your information

• Electronic contact details will be erased within three months of us finishing working together.
• Written information will be securely stored for seven years after which it will be shredded.

Your rights

• To be informed what information I will seek and hold
• To see to the information I hold about in the location where we meet.
• To amend any inaccurate or incomplete personal information.
• To with draw consent to me using your personal information

We will discuss this policy at our first meeting and if we continue to work together, we will both sign to indicate our agreement of the policy.
